I found this, so guessing it's what you said he may possible have:
Alopecia areata, an autoimmune condition where a person's own antibodies attack hair follicles, can create patches of hair loss on the scalp and can affect eyebrows and other areas where hair grows.
And then I found this:
Common causes of alopecia in children include use of chemotherapy or radiation therapy, seborrheic dermatitis (known as cradle cap), follicular mucinosis, tinea capitis, and hypopituitarism. Tinea capitis may produce a kerion lesiona boggy, raised, tender, and hairless lesion. Trichotillomania, a psychological disorder more common in children than adults, may produce patchy baldness with stubby hair growth due to habitual hair pulling. Other causes include progeria and congenital hair shaft defects such as trichorrhexis nodosa.
** I find it interesting it grows while on antibiotics. Sounds maybe autoimmune. I hate overuse of antibiotics, but has doctor tried a long-term low-dose of one during the year just to see if it makes a difference?

It was a stressful emotional time for the little guy but he is better. He eats healthy and drinks water or tea.Soda is not in the vocabulary ... He has lost all his hair and eyebrows yet has eyelashes.. During the summer time his hair starts to grow back with the heat but as soon as cold weather comes it's gone...He comes down with step throat and while on those antibiotics, his hair will start growing.
