Sounds like your friend may have got a dose of something nasty, perhaps chlamydia or gonorrhea. Maybe something less serious, but he needs to see a doctor. Unfortunately, there might be no alternative treatment. Someone else might have some suggestions, but he may have to resign himself to taking a course of antibiotics. He should, of course, take probiotics with them.
He also needs to talk with the "ladies" in question, as he could have gotten the bug from one & given it to the other. In future, if he can't keep it zipped, he should use a condom. That's what they're for.
He should try to choose just one partner, very carefully. And he should feel guilty, unless he has an understanding with his wife, & permission from her to fool around.
There are worse things than celibacy.
